Acute kidney injury (AKI): Prompt evaluation of sudden kidney-function decline related to illness, dehydration, medications, obstruction or hospitalization. Hypertension: Treatment of difficult-to-control and resistant blood pressure, including medication review and evaluation for secondary causes. Diabetic kidney disease: Coordinated care to reduce albuminuria and slow progression. Urine abnormalities: Evaluation of proteinuria, hematuria and foamy urine. Continuity of care: Follow-up after hospital, emergency-department or rehabilitation visits, with coordination among primary-care clinicians, cardiology, endocrinology and other specialists. Medication and laboratory management: Individualized treatment plans with ongoing review of kidney-safe dosing, electrolyte balance, anemia and bone-mineral health. Advanced kidney care: Education and planning for patients approaching kidney failure, including dialysis options, vascular-access timing and transplant referral. Post-transplant care: Local follow-up in coordination with the transplant center. Telehealth: Appropriate follow-up visits and lab reviews may be completed by video when clinically suitable. Our goal is simple: explain the diagnosis clearly, identify what can be improved and give every patient a practical plan.
REMOTE PATIENT MONITORING For eligible patients, remote patient monitoring allows the care team to follow health information between office visits rather than relying on a single reading taken in the clinic. What may be monitored: Home blood-pressure readings, weight trends and selected symptoms, depending on the patient’s condition and treatment plan. Why it can help: Repeated home measurements can reveal patterns, support safer medication adjustments and identify concerning changes earlier. What patients do: Use the assigned device as directed, take readings on the recommended schedule and respond when the clinical team contacts them. What the care team does: Reviews submitted information, documents trends, provides education and escalates concerns when clinically appropriate. Remote monitoring does not replace emergency care. DIALYSIS & ADVANCED KIDNEY CARE Education before kidney failure: We explain treatment choices early so patients and families have time to prepare rather than make decisions during a crisis. In-center hemodialysis: Blood is filtered at a dialysis center on a scheduled basis with ongoing physician oversight. Peritoneal dialysis: A home-based therapy that uses the lining of the abdomen as a filter and may provide greater scheduling flexibility for suitable patients. Home hemodialysis: Dialysis performed at home after training, with a treatment schedule individualized to the patient. Access planning: Timely coordination for fistula, graft or peritoneal-dialysis catheter placement when needed. Transplant referral: Evaluation can begin before dialysis. We coordinate records, referral and local follow-up with transplant centers. Ongoing dialysis care includes volume and blood-pressure management, anemia treatment, mineral and bone health, medication review and communication with the dialysis team.
