WHEN TO REFER TO NEPHROLOGY
• Rising creatinine, declining eGFR, acute kidney injury, or progressive CKD
• Persistent proteinuria, albuminuria, or blood in the urine
• Resistant or difficult-to-control hypertension
• Electrolyte abnormalities, including sodium, potassium, calcium, magnesium, or phosphorus disorders
• Acid-base disorders, including persistent metabolic acidosis or alkalosis
• CHF/cardiorenal syndrome with volume overload or difficult diuretic management
• Recurrent kidney stones or abnormal kidney imaging
• Recurrent UTIs or pyelonephritis associated with kidney disease, stones, or structural abnormalities
• Inherited or suspected glomerular kidney disease
• CKD complications, including anemia and mineral/bone disorders
• Advanced CKD requiring dialysis planning, vascular access, or transplant evaluation
