WHAT TO BRING
• Photo ID and current insurance card(s)
• All prescriptions, over-the-counter medicines, vitamins and supplements — bottles or a complete list
• Recent laboratory results, hospital records and kidney imaging, if available
• Home blood-pressure readings and recent weights, if you track them
• The names of your referring and primary-care clinicians
• Your questions, written down so we can address them

 

WHAT TO EXPECT
Please arrive 15 minutes early. We will review your health history, kidney-function trend, urine tests, blood pressure, medicines and relevant imaging. After an examination, we will explain the findings and outline the next steps for testing, treatment and follow-up. A family member is welcome to join you.

INSURANCE & TELEHEALTH
We accept most major insurance plans. Referral and authorization requirements depend on your plan, so call to confirm coverage. Telehealth may be appropriate for selected follow-up visits, lab reviews and medication discussions.
